Het gebruik van laagmoleculaire heparine bij patiƫnten met voorkamerfibrillatie

Low molecular weight heparin (LMWH) is increasingly used as a substitute for unfractionated heparin or vitamin K antagonists in patients with atrial fibrillation. Oral anticoagulation has been proven to be highly efficacious for preventing thrombo-embolic complications but the use of LMWH is based on limited scientific evidence and practice guidelines are moreover lacking. In this article we focus on two possible indications for LMWH in patients with atrial fibrillation: bridging therapy during interruption of oral anticoagulation and reconversion. The guidelines of the ESC and ACCP and the most important studies are discussed. Finally, we detail the specific protocol applied in the University Hospitals Leuven (Belgium).
