Integration of palliative care into home, nursing home and hospital care.

INTEGRATE is focused on developing and studying integration models of palliative care in the Flemish health care system. The project aims to expand the boundaries of existing research and to provide the necessary scientific evidence and insights on integration of palliative care in the various echelons of our health care system, such as in different hospital departments, home care, residential care centers. In addition, it also wants to explore the possibilities of further integration of palliative care in the community.
