Organization and effectiveness of nurse-led clinics and nurse-led consultations for outpatient and hospitalized complex chronic patients

Due to the increase of patients with chronic diseases and multimorbidity, patient care needs are becoming more complex. They need support, education, counseling and strengthening of self-management following a holistic approach. This is a role that can be adopted by nurses in care models for ambulatory and hospitalized patients. Nurse-led clinics and nurse-led consultations are care models in which nurses plan, coordinate and evaluate at least part of the care autonomously based on extensive expertise. This doctoral research aims to reach an international consensus on the core elements of nurse-led clinics and nurse-led consultations for ambulatory and hospitalized complex chronic patients, to describe the effectiveness of these care models based on international literature and to map its actual structures, processes and perceived added value. Based on those insights an intervention will be developed and tested to support nurses in conducting nurse-led clinics and nurse-led consultations.
