Given this context, we propose to study PPP in TB control in Karnataka as a case study to understand the dynamics of PPPs in India. Since 1995, GoI has made systematic effortsto involve PSPs in theRevised National TB Control Programme (RNTCP) through PPM-DOTS strategy.However, existing literature shows that, despite the promise shown by PPM models,uptake of PPM schemes by PSPs has been poor and we do not know much about the context and the factors determining the performance of PPM-DOTS.Hence, we propose to study and understand ‘what determines the performance of PPM-DOTS in India’ and also to know ‘how’ and ‘why’ certain PPMs work or fail.The proposed study will be done using the mixed methods. This is reflected in the lack of PhD level training programmes in Europe that have a focus on India. Europe lacks a knowledge base on India, the ETN addresses this by creating a multidisciplinary, multi-sectoral network bringing together six leading European universities, with six leading Indian universities, and six non-university partners (one as a beneficiary), all of whom have previously collaborated together. India's actions as a global actor impact on the interests of the EU, and India as a democratic state could potentially be an ally for the EU on international issues that will affect the welfare and security of Europe. Some of the levers to enforce patient rights are health ombudsman, litigation, patient grievance redressal systems, media and public protests. Patients rights’ are important because it has implications for the quality of health care. An effective and a functional patient grievance redressal system acts as a key accountability tool but can also galvanize structural improvements in the health system. Furthermore, practicing patient rights will reinforce ethics in health care. The published academic literature on patients rights is mostly from Europe and there is paucity of scholarly work on the implementation of patients rights’ in Low and middle-income contexts(LMIC). In India, research studies and media reports have captured the concerns on patients rights’ violations both in the public and private health care facilities. There is limited knowledge on how the existing state policies on patients rights’ are translated at the district/subdistrict and the health facility level in India. The proposed PhD will break new grounds by developing a theoretical framework to guide the implementation of patient rights policies in the health facilities from the multi-level governance perspective based on a systematic scoping review. The framework will then be tested in a LMIC setting (i.e.Karnataka, a southern state in India) using the comparative case study design. Using the framework, the PhD research will examine how the state policies in a federal political/administrative system give expression to patients rights’ in the Karnataka state. It will provide causal explanations on the existing implementation practices of patients rights’ policies at multiple levels (district/subdistrict and health facility). Policy recommendations specific to Karnataka will be formulated and validated in a stakeholder consultation workshop. The theoretical framework will serve two purposes: i) build knowledge in the domain of patients rights from a non-Western (Indian) perspective ii) generate critical policy lessons for different levels to overhaul the existing patient rights enforcement system in the state of Karnataka, India and similar other LMIC settings. "The clinical management of visceral leishmaniasis (VL) in India" "Jean-Pierre Van Geertruyden, Marleen Boelaert" "Verwaarloosde Tropische Ziekten" "To date, outside small hospital based phase III studies, no evidence exists for the field use of liposomal amphotericin B. This is particularly important considering the complexities of providing and delivering this treatment outside of tertiary level care facilities in resource limited settings. There are many questions regarding the feasibility, safety and effectiveness of using the comparatively complex liposomal preparation of amphotericin B (which requires cold chain, skilled HR able to prepare, administer intravenous infusions and monitor drug reactions) which raised major concerns considering the low level of skilled HR and capacity in the Indian rural health system. As such, I wished to demonstrate and document the outcomes of using government facilities and HR to use this treatment under routine field conditions. Prior to my work, cases of PKDL following treatment with liposomal amphotericin B had not been described in the literature, with theories that this complication would be highly unlikely following this treatment. I therefore wish to also focus on PKDL as clinical entity for which there is limited evidence in the literature regarding treatment, and offer some solutions with regards to possible treatment modalities. Additionally, although well described in East Africa, little evidence currently exists on the prevalence of HIV-VL co-infection in endemic areas of India. Considering the density and level of in-country migration from Bihar, is possible that the scale of this condition is underestimated. Noting an increasing number of patients presenting with relapsing VL who on further estimation were HIV positive, I developed a research question to determine the true prevelence of co-infection with HIV in patients routinely presenting with VL, the research question being whehter it is justified to introduce routine HIV testing for all patients presenting with VL in India – a recommendation made globally by the WHO, but not implemented in the Asian context. Co-infection with HIV was never considered to be a public health issue in the Indian context, and was not considered a threat to the elimination effort – indeed, no treatment guidelines are available for this condition, and the national programme’s position is that it not a public health problem in India. The lack of evidence is such that there exists only one report in the literature of how patients with co-infection progress over a two year period – I therefore looked to describe what the longer term progression of this chronic disease is over a 5 year period post treatment, and the relative impact of starting anti-retroviral treatment (ART). The objective is in part to develop evidence which will determine whether or not the Indian national HIV programme should recommend the use of ART in HIV-VL co-infected patients No data currently exists on patient perceptions of the variety of treatments currently available or proposed by the WHO. The main decision making processes for policy change by both WHO and regional vertical programmes have excluded patient perceptions and experiences of disease, and have been based purely on cost and effectiveness. Considering the efficacy of these new treatment modalities are similar in phase III studies, yet the different logistical requirements from the societal perspective remain very wide. For example, the societal impact of a 10 day course of combination therapy which requires the patient to travel daily to the health center to receive daily intramuscular injections is likely to be very different to treatment with a single dose therapy, or treatment with only oral medications that can be taken at home following a single point of contact with the health care provider. As such, I hope to increase the evidence base by conducting qualitative research into this domain, which will allow the policy makers to factor the patient perspectives into their decision making process. My final research question will address a lacunae in the anthropological evidence for VL in the Indian subcontinent. I plan to develop evidence of the patients experiences of suffering from VL. There is a limited amount of qualitative evidence in the literature regarding VL, and indeed none on this particular subject, which will be crucial in developing Information, Education and Communication (IEC) strategies for implementation in routine programme settings. One of the major weakenesses in the National Programme approaches towards elimination in the Indian subcontinent is the near total lack of IEC material and strategy, which is partly based on the lack of formative research that should allow better development of tools adapted to the patients understanding and experiences of disease." Hoewel het huidige BBP van 7,9% (Trading Economics 2016) positieve trends toont, zijn deze vaak gebaseerd op standaarden en indicatoren die zeer zwaar steunen op énkel de economische output van het land. Hiermee volgen ze het zgn. Neo-Classical Model of Economics (Czech 2010). Deze trends hebben de neiging om de betekenis van het culturele, sociale en natuurlijke kapitaal in de ontwikkeling van een land te ontkennen. Onderhavig onderzoek focust op de rol van het culturele kapitaal in/voor duurzame onwikkeling. We volgen hierbij de standaard-definite van UNESCO waarin cultuur zou moeten beschouwd worden als “set of distinctive spiritual, material, intellectual and emotional features of society or a social group, and that it encompasses, in addition to art and literature, lifestyles, ways of living together, value systems, traditions and beliefs “(UNESCO 2001). Vanuit historische standpunt, maar eveneens vandaag met 68,84% van de totale Indiase bevolking die leeft op het platteland (Census of India, 2011), is in India de idee “cultuur” ten zeerste verbonden met landelijke landbouw. De met landbouw verbonden activiteiten zijn belangrijke facturen in de transformatie van het Indiase Anthropoceen, waarbij de grenzen tussen landbouw, cultuur en duurzame ontwikkeling verwateren.Sinds de vroegste nederzettingen had India ontelbare en regionaal zeer diverse tradities in landbouw en boerenleven, waarvan een groot deel reeds voor altijd verloren zijn gegaan (Shiva 2000). Al deze stijlen en technieken houden rekening met de regio-specifieke noden m.b.t. klimaat en milieu. Maar bij de huidige snelle urbanisatie en toenemende bevolkingsdruk hebben zowel de regering als de industrie de neiging om deze traditionele wijsheid over het hoofd te zien, en zelfs te bedreigen, waarbij ze kiezen voor een extreme landbouw gebaseerd op technologie, met een zware prijs voor het milieu (Shiva & Holla Bhar 2001). Een mogelijke strategie om dit scenario te counteren is gebruik te maken van de wijsheid en weerbaarheid die eigen is aan een rurale gemeenschap, om het pad naar een lange-termijn, duurzame ontwikkeling in te slaan. Het onderzoeksproject onderzoekt deze mogelijkheid kritisch aan de hand van de casestudy van de historische koninklijke opbrengsttuinen die over heel de Bundelkhandregio kunnen gevonden worden. De historische regio Bundelkhand ligt in het centrum van India, en telt nu 13 districten over twee Indiase deelstaten (zeven districten in het zuiden van Uttar Pradesh en zes districten in het noorden van Madhya Pradesh). Hiermee omspant de regio ca. 70.000 km² met een bevolking van 15,5 miljoen inwoners (Census Organization of India 2015). In het Chhatarpur District van Bundelkhand bevindt zich het stadje Rajnagar, het administratieve centrum van dat deel van het district, en een tweelingsstad van Khajuraho, dat beroemd is wegens de middeleeuwse tempels die op de lijst van het UNESCO Werelderfgoed staan (UNESCO 2016). In 1998 bestelde de deelstaatregering van Madhya Pradesh een rapport voor een strategie voor het behoud en de duurzame ontwikkeling van de Khajuraho regio. Deze regio werd in dit rapport geografisch bepaald, en omvat ook Rajnagar, waar zich deze opmerkelijke productietuinen bevinden. (Khajuraho Planning Team 1998). De tuinen in Khajuraho/Rajnagar hebben alle dezelfde kenmerken : ommuurd, met een kleine Shiva-tempel, een buitenhuisje (kothi), crematie platformen (samadhi), verschillende waterputten (sommige met trappen), irrigatiekanalen; de oppervlakte varieert van ca. 120 tot ca. 250 are. De geschiedenis van deze tuinen is nagenoeg onbekend, evenals de reden voor zulke ongelofelijke concentratie van 15 tuinen in Rajnagar zelf. Deze tuinen werden in de tweede helft van de 18de eeuw/begin van de 19de eeuw aangelegd door de koninklijke familie van Chhatarpur als opbrengsttuinen voor groenten, bloemen en fruit. De hypothese voor Rajnagar is dat elke tuin verbonden is met een erfprins, die – na zijn regeringsperiode als koning – in de tuin zou gecremeerd zijn, evenals mogelijks zijn naaste familie (Babbar & Robberechts 2007). Naar verluidt kwamen na de Indiase Onafhankelijkheid (1947) de meeste tuinen in privé-handen als geschenk van de koninklijke familie.Met hun specifieke irrigatie- en landbouwmethoden vormden deze tuinen een uniek duurzaam en zelfonderhoudend micro-ecosysteem in een regio die onderhevig is aan een snelle urbanisatie wat een bedreiging vormt voor hun voortbestaan. Slechts in 1998, in het vermelde Rapport over de Khajuraho Regio, werden de tuinen voor het eerst erkend als historische landschap, en niet als gewoon een landbouwveld.Hoewel de streek historisch gezien onderhevig is aan droogte-periodes, werden er de laatste jaren nieuwe water-intensieve landbouwtechnieken geïntroduceerd die tot grotere droogte-problemen leidden; de weerbaarheid van de lokale gemeenschap blijkt onvoldoende om hierop te reageren, en leidde tot verschillende zelfmoorden van landbouwers die gebukt gingen onder te zware leningen (Shiva & Holla Bhar 2001). Via de case-study aanpak kan de inherente gemeenschapswijsheid en –weerbaarheid (Upadhyay 2015) ingezet worden om te kunnen omgaan met de klimaatverandering (Young 2012). Dit betekent dat deze tuinen – om hun betekenis te begrijpen –niet kunnen worden bestudeerd als geïsoleerde culturele erfgoedeilanden temidden van nederzettingen, akkers en weiden. Om hun rol in de huidige omgeving te bepalen  wordt de gehele Khajuraho Heritage Region (Khajuraho Planning Team 1998) onderzocht, inclusief het landbouw-landschap, de waterhuishouding, Rajnagar als nederzetting en de nabij gelegen toeristische bezienswaardigheden.Het belangrijkste doel van het onderzoek is bijgevolg de analyse van hoe/of behoud van historisch landschap kan bijdragen tot de duurzame ontwikkeling van de regio. De doelstellingen die tot het vermelde doel moeten leiden zijn : vooreerst de analyse van de sleutelproblemen en bekommernissen m.b.t. behoud van historische landschappen; ten tweede de analyse naar de rol van de sleutel-stakeholders en de gemeenschapsparticipatie in het conservatie-proces van historische landschappen op globaal niveau; ten derde de analyse van de brug tussen het (economische) ontwikkelingsbeleid en de huidige beschermingssystemen voor historische landschappen in India. Nadien probeert onderhavig onderzoek de planning en context van de 18de-eeuwse koninklijke productietuinen in Rajnagar, Bundelkhand, te begrijpen en alle waarde-categorieën van deze tuinen op te stellen, binnen de bredere context van de veranderende landbouwomgeving van Rajnagar. En tenslotte probeert het onderzoek aan te duiden hoe de tuinen van Rajnagar kunnen bijdragen tot de duurzame ontwikkeling van de regio.Vanuit de aard van het onderzoeksproject moet een mengeling van zowel kwalitatieve als kwantitatieve onderzoeksmethoden worden toegepast. Na literatuurstudie, gebruikt de auteur de zgn. grounded theory analysis  om eerst de reikwijdte van meningen en waarden van het landschap in de bredere context van de veranderende landbouwomgeving in Rajnagar te identifiëren, doorheen alle respondenten d.m.v. semi-gestructureerde interviews. De monsterneming van deze interviews wordt gebaseerd op de sneeuwbal-techniek die zelf gebaseerd is op het sociale netwerk van de uitgekozen geïnterviewden.De resultaten die aldus behaald worden, worden bijeengebracht in de vorm van een model, om  daarna op hun werkzaamheid getest te worden op een andere, nabijgelegen site met gelijkaardige geografische en historische context (d.i. met gelijkeaardige tuinen). In de veronderstelling dat het resultaat hiervan positief is, kan het ontwikkelde model (met bepaalde aanspassingen) de basis vormen in andere scenario’s voor een door cultuur geleide rurale ontwikkeling." Hun verspreiding heeft geleid tot de creatie van meerdere versies, aangepast aan verschillende literaire, taalkundige en culturele domeinen van het premoderne India, waardoor het nationalistische proces van de homogenisering van de canon in vraag wordt gesteld. Daarom bieden deze teksten inzicht in de composiete literaire geschiedenis van India, getuige de deelname van Indiase soefi-dichters aan praktijken van dialoog en accommodatie. Deze bronnen zijn echter onvoldoende bestudeerd door literatuurhistorici, die hun relevantie voor hedendaagse debatten over taal- en identiteitspolitiek in India vaak niet erkennen. Dit project wil deze leemte mee opvullen door te focussen op een bepaalde tekst van deze traditie, Usman's Citravali (1613), en de reproductie ervan in verschillende culturele, literaire en taalkundige netwerken. Door een close reading zal ik aantonen hoe het verhaal is ingebed in de kosmopolitische en religieuze competitieve praktijken van de premoderne niet-westerse wereld. "Management of Visceral Leishmanisis in Muzaffarpur, Bihar, India" "Shyam Sundar, Jean-Pierre Van Geertruyden, Marleen Boelaert" "Verwaarloosde Tropische Ziekten" "Human visceral leishmaniasis (VL), also known as Kala-azar (KA), is a major health problem and cause of mortality, particularly in developing countries. Leishmania organisms are endemic in more than 80 countries and 350 million people are considered to be at risk (Desjeux P 2004). More than 90% of the cases worldwide occur in Indian subcontinent, Brazil and Sudan. In India, 90 percent of the VL cases are reported from a single state, Bihar. The disease is fatal if left untreated and case fatality ranges from 5 to 15% even with treatment (Boelaert M et al 2000). In Indian sub-continent humans are the only known reservoirs of the parasite.In India, a centrally organized and sponsored VL Elimination Initiative was launched in endemic areas in 2005. The National Kala-azar elimination program is based on vector control by Indoor Residual Spraying (IRS) of houses and cattle sheds; and early detection and treatment of cases in VL endemic districts. The program relies on the public primary health care system at district and block levels (Hasker E et al 2010) and it is monitored using passive surveillance.The passive surveillance system in use fails to record the majority of cases started on treatment because they are treated by the private health sector and does not provide reliable information on treatment outcomes, even for those treated within the public health system. Inadequate surveillance is causing lack of information on disease burden which in turn magnifying the severity of the problem.
