Chapter I assesses the quality of microscopic diagnosis of malaria countrywide in the Democratic Republic of the Congo by an External Quality Assessment (EQA) session organised through the combined networks of the national malaria and tuberculosis control programs. The study documents the competence of the microscopic diagnosis of malaria among 174 laboratories in DRC and assesses the quality and procurement policy of the Giemsa stain solutions. Chapters II addresses quality control issues of malaria rapid diagnostic test (RDT) as evaluated by EQA in a non-endemic setting (Belgium and Luxembourg, 123 participants). In addition to evaluating the analytical competence of the participants and the performance of the RDT kits, RDT package instructions are evaluated and the place of RDTs in the diagnostic strategy is assessed. Chapter III assesses the competence of reading and interpretation of malaria and HIV RDTs by endusers in an endemic setting by means of EQA. Short cases with high-resolution pictures are presented to end-users, who are asked to reply by SMS. The cases focus on problems and errors in reading and interpretation of malaria and HIV tests, as well as on the use of the diagnostic algorithm for the HIV tests. Chapter IV assesses the visual acuity of end-users of malaria and HIV RDTs in an African field setting. Poor visual acuity from the end-users may affect RDT reading, especially since up to a third of malaria RDTs show faint and weak lines. Simple but unnoticed and uncorrected presbyopia (an accommodation deficit resulting in near vision failure) has been reported to be more frequent in African population as compared to Western populations. The prevalence and degree of presbyopia and its effect on RDT readings will be assessed among end-users in RDC field settings. Chapter V describes the adaptation and validation of the Dried Tube Spot technique (DTS, used to prepare control samples for HIV RDT evaluation) to other applications in RDT testing including malaria, leishmaniasis and human African trypanosomiasis. The DTS system will be converted to the use of whole blood samples, assessed for heat and humidity stabilisation, and subjected to pilot EQA sessions in field settings." Individuele verschillen tussen patiënten kunnen in kaart gebracht worden door de analyse van bepaalde biomerkers in medische laboratoria. Aan de hand van de status van deze biomerkers in het tumorgenoom wordt er een therapieplan opgesteld. Hierdoor is het van uiterst belang dat de kwaliteit in de medische laboratoria te allen tijde gewaarborgd wordt. Het educatieve karakter van externe kwaliteitscontroles (EQA’s) kan deze laboratoria helpen om de kwaliteit van het diagnostische proces verder the optimaliseren.De European Society of Pathology organiseert in samenwerking met de groep Biomedische Kwaliteitszorg van de KULeuven op jaarlijkse basis externe kwaliteitscontroles (EQA’s) voor long- en colonkanker. Uit cijfers blijkt dat nog steeds 30% van de deelnemers niet in staat is om tien toegestuurde stalen correct te analyseren, alsook dat er meer fouten gemaakt worden na de introductie van een nieuwe test. Hierdoor komt de patiëntenveiligheid in het gedrang.Dit project zal een longitudinale aanpak hanteren om data omtrent de soorten fouten, hun oorzaken en frequentie te identificeren, te classificeren, en te analyseren voor long- en colonkanker. Daarenboven zal de efficiëntie van verschillende correctieve maatregelen die deel uitmaken van het kwaliteitssysteem (QMS) geëvalueerd worden. Doorheen het project wordt er ook telkens een focus gelegd op fouten die voorkomen tijdens de implementatie van nieuwe, opkomende technieken in medische laboratoria, zoals next-generation sequencing. De informatie zal verzameld worden voor de deelnemende EQA laboratoria, alsook voor de routine praktijk tijdens de opvolging van een specifieke selectie van Europese laboratoria. Tot slot worden deze data geanalyseerd met als doel inzicht te verwerven over de invloed van EQA en QMS op strategieën om fouten te reduceren. Er is nood aan aanbevelingen en een betere structuur ter ondersteuning van de laboratoria bij het optimaliseren van hun resultaten of bij het implementeren van nieuwe tests.Het project start met een longitudinale studie met het doel om kwaliteitsindicatoren te identificeren. About 346 million people have diabetes and more than 80% of diabetes deaths occur in low- and middle-income countries. Diabetes deaths will double between 2005 and 2030. In Morocco, non-communicable diseases are responsible for 55.8% of the overall burden of diseases and diabetes prevalence has been estimated at around 7% of the population The response to non-communicable disease like diabetes is complex and health systems are not prepared to tackle diseases that do not heal and need to be treated and managed continuously as part of the daily life of patients and their families. In response to this challenge, the World Health Organization has developed the Innovative Care for Chronic Conditions Framework. This framework is centered on a triad of partnerships between the patient, the health care team and the community. Even if community action has been recognized by the Ottawa charter as a pillar of health promotion, the debate over the real capacity of community action to empower people and improve health is still relevant given that patients’ empowerment is a major issue to control NCD. In the Moroccan context, we are experiencing a proliferation of civil society organizations that support diabetes patients, but this phenomenon has not been investigated although these organizations are increasingly recognized as an important actor widely represented in the different regions of Morocco. Our research question regards the role of diabetic associations on the empowerment of diabetic patients. This research question could be divided into two research sub-questions: • To what extent do diabetic associations contribute to diabetic patients’ empowerment? and what characteristics of associations make them more empowering than others ? • To what extent (under which conditions) does patient’s empowerment lead to improving clinical and quality of life outcomes? We will try in this protocol to answer to the first research sub-question. Our objectives are to describe the characteristics (including activities) of the diabetic associations and analyze the different factors that determine their capacity to be empowered. We will conduct a qualitative study with a descriptive and interpretative perspective, in the prefecture of Marrakesh and the province of Al Haouz. Our study population will consist of the leaders and the members of diabetics associations, health professionals dealing with diabetics associations from the private and public sectors and the other partners working with diabetic associations (Politicians, territorial administration, and Pharmaceutical industry, Regional department of health, Diabetes prevention and control program managers). We will explore the following dimensions: The determinants of the creation of the association, the evolution of the association over time, the reasons of becoming a member of such diabetes associations, the mission and goals of diabetics associations, the organizational characteristics of associations (participation and leadership, resource mobilization, role of external agents not directly related to diabetes in association, activities and accomplishments of associations, collaboration of the association with health services and with other partners such as associations, local authorities, elected representatives, pharmaceutical industry). We will also investigate through a self-assessment of the association leaders the association’s realizations and the perception of diabetic patients about the association. Our work will help enabling health system to be more proactive in working with community actors acting in the field of diabetes by clarifying their roles and integrating them in the management of diabetes and the others non communicable diseases." In dit project bestuderen we het welzijn van vleeskippen, respectievelijk geproduceerd in Vlaanderen en in Zuid Brazilië (vleeskippen bestemd voor de Europese markt). We gaan na welke verschillen qua dierenwelzijn er zijn tussen beide regio’s. Tegelijkertijd evalueren we het in Europa ontwikkelde Welfare Quality® protocol om het welzijn van vleeskippen te monitoren. De laatste decennia heeft de pluimveehouderij verschillende ingrijpende intensiveringen ondergaan.  OnderzoeksaanpakGebruik makend van het Welfare Quality® protocol brengen wij het welzijn in kaart op willekeurig gekozen Vlaamse en Zuid-Braziliaanse vleeskippenbedrijven (telkens 10). Ook de bijhorende slachthuizen worden bezocht door een gemengd team van zowel een Vlaamse als Braziliaanse onderzoeker die een training hebben ondergaan om het Welfare Quality® protocol toe te passen. Beide observatoren voeren simultaan maar onafhankelijk van elkaar de welzijnsevaluaties uit. Deze evaluaties vinden plaats op het pluimveebedrijf tijdens de laatste week voor slacht en in het slachthuis. Binnen Brazilië varieert de wijze waarop vleeskuikens geproduceerd worden in functie van de afzetmarkt. Daarom is beslist om te focussen op Braziliaanse bedrijven die exporteren naar de Europese markt.    Relevantie/ValorisatieDit project dat de welzijnsstatus van geïmporteerd kippenvlees uit Zuid-Brazilië vergelijkt met die van lokaal geproduceerd kippenvlees uit Vlaanderen legt de sterktes en zwaktes bloot van de productiesystemen in beide landen. Diergeneeskunde" "Use of animal based measures for the assessment of dairy cow welfare" "Rundveehouderij, Dier" "Centrale onderzoeksvraag/doelHoe kunnen we het welzijn van (Europese) melkkoeien op het niveau van het individuele dier in kaart brengen door bestaande registers en metingen op het bedrijf selectief te herwerken tot een betrouwbare indicator voor welzijn op epidemiologische schaal? Deze onderzoeksstudie in opdracht van de Europese Unie wordt door een groot aantal Europese partners uitgevoerd, waaronder ILVO. Het is de bedoeling een correcte vergelijking  te maken tussen diergebonden metingen, tussen landen, tussen regio’s en tussen productiesystemen. Dier-gebonden metingen worden langzaam maar zeker geïntroduceerd in de EU wetgeving. De richtlijn voor vleeskippen (Council Directive 2007/43/EC1) en de richtlijn voor het slachten van dieren (Council Regulation EC No 1099/20092) waren de eerste richtlijnen waarin deze gebruikt werden. De EU strategie voor de bescherming en welzijn van dieren (2012-2015) voorziet een nieuwe EU wetgeving waarbij wetenschappelijk gevalideerde dier-gebonden metingen opgenomen zijn. OnderzoeksaanpakElke partner is verantwoordelijk voor een specifiek deel van het project. De eerste stap is het inventariseren van de grootste welzijnsproblemen bij melkvee. Dit zal in termen van prevalentie en impact op het dier uitgedrukt worden. Vervolgens kunnen de dier-gebonden metingen die deze problemen meten geïdentificeerd worden. We gaan bij voorkeur op zoek naar relevante metingen die op heden reeds verzameld worden in het veld. Het resultaat is een set dier-gebonden metingen waarmee de meest relevante epidemiologische welzijnsparameters worden gecoverd. De selectie wordt geëvalueerd op validiteit ( inclusief specificiteit), reproduceerbaarheid en herhaalbaarheid. De onderzoekspartners bekijken op welke wijze en of de diergebonden metingen onderling, per land, regio en productiesysteem vergeleken kunnen worden. Vervolgens leidt ILVO de ‘pilot’ test waarbij de data samengevoegd en ingevoerd worden in het EFSA Data Verzamel Platform. Relevantie/ValorisatieMet dit project worden de eerste stappen gezet in de richting van het implementeren van diergebonden metingen van het welzijn van melkkoeien in Europese richtlijnen. Dit door in kaart te brengen welke metingen routinematig verzameld worden en welke de grootste welzijnsproblemen onder melkvee aantonen. We linken welzijnsscores met data over huisvesting, bedrijfsvoering, en gegevens uit bestaande databanken, om kandidaatmerkers te selecteren van bedrijven met verhoogd risico op welzijnsproblemen. We bepalen de beste en economisch meest haalbare frequentie en methode van dierenwelzijnsevaluaties. We gaan na hoe we m.b.v. geschikte marketingstrategieën de consument kunnen motiveren om een meerprijs te bepalen voor diervriendelijke melk. We onderzoeken in welke mate de melkveehouder bereid is om verbeterstrategieën te implementeren om tot een meer diervriendelijke maar toch rendabele melkproductie te komen.  Via een participatief proces krijgen we zicht op de percepties van stakeholders omtrent alle bovengenoemde punten.Relevantie/ValorisatieVoor de burger is dierenwelzijn een inherent en belangrijk aspect van kwaliteitsvolle voeding. De toegenomen maatschappelijke vraag naar meer dierenwelzijn wordt vooral beantwoord door wettelijke minimumnormen voor huisvesting en management. In dit project pogen we aan deze maatschappelijke vraag tegemoet te komen door private initiatieven vanuit de sector zelf, initiatieven gericht op een meerwaarde door rechtstreeks in te spelen op de behoeftes van de consument en gestoeld op wetenschappelijk gefundeerde methodes om dierenwelzijn te monitoren. Een in de praktijk bruikbare methode om welzijn te meten moet enerzijds correct zijn maar anderzijds ook snel, goedkoop, transparant en informatief. De extra kosten voor het op grote schaal toepassen van dierenwelzijnsmetingen en voor meer diervriendelijke productieprocessen proberen we terug te winnen door  m.b.v. geschikte marketingstrategieën de consument te motiveren om er een meerprijsvoor te bepalen.
