Evidence convincingly shows that the reduced mortality rate over the last two decades after heart, liver, kidney and lung transplantation (Tx) is exclusively attributable to survival gains in the first year post-Tx, but that long-term survival does not mirror these immediate post-Tx successes 1. Increasing long-term survival is therefore a major goal within Tx research and clinical practice. Self-management emerges as a corner stone to achieve this
