Re-engineering care in AlloSCT: an integrated model of care facilitated by eHealth – The SMILe study - Belgium.

Allogenic stem cell transplantation (alloSCT) is a curative treatment for various hematological diseases and has led to an increasing number of transplant survivors. However, alloSCT recipients have a high risk of developing complications and late effects, making them often very comparable to chronically ill patients. Current care models are predominantly acute care-oriented and lack an integrated care approach such as the Chronic Care Model (CCM). The CCM focusses on 5 essential aspects of health such as the importance of patient self-management support, health education, the availability of clinical information systems, clinical decision support systems and an efficient delivery of care (Bodenheimer et al) all of which can be enhanced by eHealth (Gee et al). This results in truly engaged and educated patients who are empowered to go in interaction with a prepared proactive team. In several chronic diseases, such as diabetes, such an approach has been shown to improve long-term outcomes (Bodenheimer et al). The SMILe project (SteM Cell TransplantatIon faciLitated by eHealth Technology) aims to re-engineer care for alloSCT patients to better fit an integrated care model facilitated by eHealth (SMILe-ICM). It is a multi-center project running in an international scientific consortium including the Universities of Basel, Freiburg, Zurich and Leuven, and relies upon implementation science, behavioral science, and computer science. The SMILe project Belgium will consist of 2 phases: phase A, the development of the SMILe-ICM intervention adapted to our Belgian context, and phase B, the implementation and evaluation of the intervention in our local alloSCT center. During phase A, a thorough contextual analysis will be conducted to gather quantitative and qualitative information covering the target context and choose an appropriate theoretical implementation framework. Then, the adapted Belgian SMILe-ICM prototype will be further evaluated in phase B following a hybrid effectiveness trial design.
