Integrated and long-term care for adults and the elderly - design of a project context (R-12031)

Recently, the client centered approach has become more applied within the care setting for persons with a disability. However, the coordination and cooperation between both the subsidizing authorities and the different care facilities appears to be challenging. Hence, affecting the quality of care and consequently the quality of life of persons with disabilities. With this research project, the Flemish Government aims to create a framework for the project call concerning integrated care for adults and elderly with long-term care and support needs. Therefore, following aspects need to be defined: An experimental framework for clients and initiators should be developed. In addition, a definition of the population and the concept of 'integrated care' needs to be well described and determined. Finally, in order to monitor and evaluate the effectiveness of integrated care in practice and to identify areas for improvement, a valid set of indicators is needed. Methodologically, the project is shaped by the design science method, aiming to develop complex interventions and frameworks. This multidisciplinary project consists of three work packages, operational from July 2021 to December 2021, in which we aim to find an answer to the research questions, as described above. Moreover, this project is carried out across two universities (VUB and UHasselt) because of the specific expertise requested.
