Implementing multidisciplinary heart failure management in primary care

Heart failure (HF) is an important health problem. Most chronic HF management occurs in primary care. Although guidelines exist, there is an important implementation gap in current HF care in Belgium. The aim of this research project is to study these gaps and possible remedies such as audit and feedback in a process evaluation of a dynamic regional multidisciplinary HF intervention.
