Integrated care for persons with Chronic kidney disease

In this project we will make and implement an enhanced care-model for patients with Chronic kidney disease. This model will be based on a combination of existing knowledge and experiences with the implementation of CKD guidelines, audits and benchmarking and peer meetings to support quality improving actions
