Towards integral aftercare for cancer patients: development of a care path after cancer treatment

Cancer and its treatment can have an impact on the physical and long-term psychosocial well-being of the patient and his environment . Multiple national and international studies indicate that people after cancer treatment are often not appropriate receive care . Several experience-oriented care models have been developed for the organization of aftercare . The emphasis is often on a proactive approach late effects, reintegration follow-up and shared follow-up with frontline workers. There is no clear evidence-based guidelines and there is no alignment with the Flemish context insufficient . A recent policy note also affected the absence of one in Belgium coordinated and interprofessional aftercare offer. The development of a care path is possible meet the challenge of making aftercare part of total care for every patient.
