G-COACH: Geriatric CO-mAnagement for Cardiology patients in the Hospital

Given the ageing of the population, geriatric wards being tested to the limits of their capacity, and an increasing shortness of geriatricians in the hospital, there is an urgent need to explore innovative care models that are able to sustain or improve healthcare outcomes for older adults. The consultation team care model failed to show significant effects on patient and hospital level, the main limitation being its recommendation-based character. The G-COACH project aims to study the efficacy of an alternative geriatric co-management model which has only been tested to a limited extent and almost exclusively in an orthopedic population. G-COACH will introduce a co-management intervention for cardiologic/cardiac surgical patients conducted by geriatric nurse practitioner clinical nurse specialists, geriatricians and cardiologists/cardiac surgeons. The process and outcome indicators in this exploratory study will be based on a systematic literature review and an international Delphi-study.
