D-SCOPE: Detection - Support and Care of Older People in their Environment.

Health care is increasingly being confronted with older people who suffer from a multitude of biopsychosocial problems ('frailty, or vulnerability as a result of a mix of problems), but prefer to grow old in their own home environment. If such fragility is not timely detected and accurately supported, this can lead to a decline in the quality of life of the elderly and its environment, and the independent home living is also threatened. Although there are numerous support channels, there is still a lack of systematic and timely detection of frailty in the elderly and active guidance/empowerment to the right support in the immediate vicinity of the elderly.
